WHAT WE NEED TO PROCEED WITH YOUR AUDIT

 

 

  1. Copies of your telephone and/or utility bills:
     

    • Complete copies of your gas, electric, water, sewer and waste  bills for the past twelve (12) months
       

    • Complete copies of your telephone -- both local and long distance -- bills for the most recent two months
       

      • Please Note: Any additional billings provided will help ensure a more thorough audit.

         

  2. A blank, signed Letter of Agency typed onto your company's letterhead:
     

    • Follow the Letter of Authorization link at the bottom of this page for an example
       

    • This Letter of Agency will give the utility companies authorization to provide information to Eric Ryan

     

  3. Copies of any Contracts or Agreements:
     

    • Any contracts or agreements between your company and any telephone and/or utility companies are critical to the audit process

     

  4. Copies of your state and federal sales tax exemption forms (if applicable)

     

  5. Complete the Client Information Form:
     

    • Client Information Form can be found by following the Client Information Form link found at the bottom of this page
